Partnerships for Families
SHIELDS for Families, in collaboration with its partners, proposes to implement ACT: Achieving Change Together, a Partnership for Families Program, for high risk families referred from the DCFS Wateridge Office in SPA 6 (zip codes 90001, 90002, 90003, 90011, and 90044) and high risk pregnant women referred from the community.  The program has three primary components: Direct Services, Internal Capacity Building and External Capacity Building.  The Direct Service Component is designed to provide a community-based, one-stop, comprehensive, collaborative and culturally competent continuum of services.  All services incorporate the Family Support Principals and are provided using a family strength-based approach.  In addition, services are offered in English and Spanish, are home, community and/or centered based and include: case management, concrete services, linkage to intensive services, family education and support, early child care and education and referrals to auxiliary supports.
