Funding For:
Healthy Babies/Happy Moms
The Healthy Babies/Happy Moms program’s long-term outcome is to improve the health outcomes of women, children and their families in the cities of El Monte, La Puente and Baldwin Park within SPA 3. The project is a five-year collaborative effort between Citrus Valley Health Partners (lead) and Community Outreach for Prevention and Education (COPE) and Getting Enrollment Moving (GEM). The program’s primary services include: (1) culturally and linguistically competent services for the Latino and Asian populations. These services include educational information, through community outreach regarding what healthcare services are available and how they can access them; (2) assistance with enrollment in appropriate health insurance program if eligible; (3) pregnancy testing, vision and hearing screenings including referrals for follow-up care and health education provided by bilingual health care professionals (4) Comprehensive Perinatal (CPSP) and Obstetrical services provided by bilingual healthcare professionals for the most at-risk pregnant mothers; (5) referrals to appropriate culturally competent primary care physicians in their neighborhood to ensure their connectivity to long-term “medical homes”; (6) and transportation to the Healthy Babies clinic, obstetrician, and hospital for required services.
