Request for Qualifications:

First 5 LA is issuing this Request for Qualifications (RFQ) to businesses interested in working with our Public Affairs department as a non-exclusive Printing Contractor. The primary function of the printer will be to supply First 5 LA with both digital and offset print production services.

How to Apply:

The application period is now closed. Individuals and organizations that wished to apply for the RFQ must have submitted a completed online application to First 5 LA no later than 5 p.m on April 1, 2010. Following a review of all submitted materials, qualified applicants will be invited for an in-person or phone interview beginning April 5, 2010.

Important Documents 

All applications must be received electronically.  The following documents should be included in your application:

• Application Cover Letter (2 pages maximum. 1 original, signed in blue ink)*
• Expertise and Experience as a Printer (5 pages maximum)
• Sample Quotes (For wall calendar, presentation folder, peel off stickers - in single document) 
• References (maximum of 5)
• Signature Authorization Form (doc) (Appendix C)*
• Agency Involvement in Litigation and/or Contract Compliance Difficulties (pdf) (Appendix D)*
• Sample Board Resolution (pdf) (if applicable)
• W-9 (pdf)  (Appendix G)
• Business License
• Supporting Documentation (Four (4) copies of all samples and any supporting documentation the applicant may deem appropriate)
• Sample Contract (pdf)**

*In addition to the electronic version of the Signature Authorization form and Agency Involvement in Litigation, please mail an orignal copy, along with any supporting documentation or samples in print form you deem appropriate to:
